1. DATES: Our Outreach calendar shares the one diary, so please get those dates correct!
2. LOCATION: Be aware that the address confirmed by email will be the Swan Hill "3 Gray Street" clinic address, this is because it is a Mobile Outreach Service from this main address for Private Health / Medicare claiming reasons. The locations are as advertised on our website & Google calendar.
3. APPOINTMENTS: If you are unable to find an available / suitable date and time, please call our reception directly for support.
4. CONFIRMATIONS: All Outreach appointments booked online or otherwise will be confirmed in the week prior to your visit with us. Please confirm ahead of time.
5. NON-ATTENDANCE: Non-attendance to a scheduled Outreach appointment will incur a 100% Fee due to operational expenses to provide mobile service. By booking an appointment, you agree to these terms.
